The team at 4 Success Schools is dedicated to providing quality customer service to school districts.
- Day Program for ED, SLD, OHI, MIMR, TBI, and Autistic.
- OSS- Out of School Suspension. Up to 10 days available for suspended students.
While the home school district retains the student and records, we feel it is our responsibility to keep the home district informed and updated.
The following is a list of services we provide to school districts at no extra charge.
Daily
- Medication monitoring and documentation
- Documentation of attendance in compliance with ADE regulations
- Time of incident physical intervention documentation and CPS reporting
Monthly
- MIPS documentation and submission
- Submission of monthly attendance records, including 40 day and 100-day census
- Submission of monthly behavior modification records on every student
- End of month billing
Quarterly
- Mid quarter Progress Reports
- End of quarter report card and IEP progress documentation
Annually
- Notification to district personnel of upcoming IEP’s and MET’s
- IEP scheduling, writing and meetings
- Compliance with all district transition, promotion, and graduation requirements
- Academic achievement testing
- Organizing, administering and submitting all AIMS and Stanford 9 tests.
We store all students’ records for 5 years.
